2.2 Living will The compilation of a living will offers you the opportunity to express your wishes in terms of organ donation and to give permission to disconnect machines that are used to keep you alive if you are no longer able to communicate. This is an informal document containing your wishes, but it is not accepted in a court of law. This document may comprise as much information as you require. Remember to inform your spouse, children and medical doctor about your wishes as contained in your living will.  NB: Wishes pertaining to funeral The information contained in this document will assist the funeral director to fulfil your last wishes concerning your funeral/memorial service. Remember to review the document from time to time and allow discretion for your loved ones to take some of the decisions – they would want to be involved in the planning process when the time comes to say goodbye.  Organ donation It may be your wish to assist other people after your death by donating your organs. Make sure your family and general practitioner are aware of your wish to assist you with this process. (Inform the medical team and grant authorisation.) Contact SA Medic Alert, PO Box 4881, Cape Town 8000, where you can get an organ donor card with registration, to inform your medical team. You can also contact the Organ Donor Foundation directly. ANNEXURE 2 3. Important documents 3.1 Funeral policy - Is your current policy sufficient to cover funeral costs? - If you do not have a policy, have you made provision for the cost of a funeral? Are these funds available to your loved ones? - Have you decided whether you want to be buried or cremated? Do your loved ones know what your preference is? - Do you prefer to have flowers at your funeral, or do you prefer that donations be made towards a specific institution? # See Annexure 3 – My last wishes. 3.2 Other  Financial advisor Include his/her name and contact details.  Policies It is important to keep the original contract documents on file as proof of ownership and to indicate at which companies the policies are being held.  Retirement annuities The original contract document must be kept on file, along with the latest statement or correspondence from the relevant company with whom the annuity is held.  Copy of your salary advice (if not retired) If you have not yet retired, you must keep a copy of your latest salary advice on file. If you have already retired, this is not applicable.  Short-term insurance contract It is important to ensure that your short-term insurance cover is up-to-date and sufficient to satisfy your current circumstances.
